This membership plan is not an insurance policy; it operates as a discount program.

Please note the following details about the plan:

• It cannot be used for dental treatments already covered by your insurance (i.e., no double coverage).

• You may use it for additional cleanings, exams, X-rays, even if you have dental insurance.

• The plan is not applicable to worker’s compensation claims.

• It covers only the services provided by Dr. Charmi Solanki, and the in-house team.

• It does not include referrals or other specialists.

• The membership plan is exclusively valid at Legacy Periodontics & Implant Center.

• It cannot be applied if other insurances, such as automobile, homeowners, or medical insurance, are covering the dental care.

• If you fail to provide sufficient notice when rescheduling an appointment, that appointment will count as one of your allocated visits.

• The plan does not include discounts on office-sold products like post op kits.

• It is non-refundable.

• Membership fees are subject to annual changes.

• Payment for services must be made at checkout for the discount to apply.
